If you are taking regular medication please try not to run out at weekends and holiday times. Repeat prescriptions can be ordered by post, via this website, in person, or by telephone between 8.00am & 11.00am only. Prescriptions will be ready for collection after 2.00pm the following day. To use the repeat prescription facility on this website you must first visit the surgery to collect an on-line registration form and pin number.

Disabled Facilities

Wheelchair ramp and ground level consulting are available. We have a wheelchair on the premises. An Induction Loop is fitted for the hard of hearing.

Emergency Cover when the Surgery is Closed

Out of hours cover is arranged by Leeds Primary Care Trust. If you need urgent medical help because a new illness has started or an existing one has become worse.

Or Tel: 0345 605 9999 to speak directly to the Out Of Hours Service. YOU SHOULD DIAL 999 If the patient has:

  • Severe pain, especially in the chest.
  • Severe blood loss.
  • Prolonged unconsciousness.
  • Extreme difficultly in breathing.
  • A prolonged fit.

The practice is registered under the Data Protection Act and strict patient confidentiality is observed at all times. Please do not ask us for information about anyone else – we are not able to give it. All employees have signed a confidentiality agreement. Where information is stored electronically we comply with the current legislation. In the course of everyday patient care and administration your data may be shared, in confidence and only if necessary with other specific NHS organisations.

Patient Responsibilities

We will treat you with respect, care and courtesy and we ask that you do the same. Please be on time for appointments and cancel appointments you cannot attend. It is important that you inform us promptly of any change of address, name or telephone number. If you are under the care of a hospital or waiting for an appointment please notify that organisation too.Patients who are abusive or violent will be removed from our list and Leeds Primary Care Trust will make alternative arrangements for their health care.

Suggestions and Complaints

If you have any suggestions for improving our service the doctors and staff will be glad to consider them. We realise that things do go wrong sometimes. If you think this has happened to you we have an internal complaints procedure. Please contact our Practice Manager, Mrs Deborah Hollings, so that we can try and sort out your problem and perhaps prevent it happening again. This does not affect your rights to pursue a more formal complaint.
